What Are Dental Implants?

General Parts of a Dental Implant


A natural tooth has two main parts:








Root : which is in the bone and holds the tooth in place


Crown : the visible teeth part most people think of as the tooth










A dental implant is a titanium screw that is carefully positioned into the jawbone to act as a foundation for supporting artificial teeth, crowns, bridges or dentures.


A dental implant can be viewed very smiliar to that of a natural tooth. It has following components
               
                                     
Implant Posts: An implant post is a titanium "screw" surgically inserted into the bone of the jaw that act as a replacement root for the missing tooth. Implant posts are normally threaded on its external surface and the internal aspect of the implant is usually a hollow well with screw threads. Titanium is chosen for this purpose because titanium is inert, biocompatible and integrates with the surrounding jaw bone with minimal risk of infection.


Abutment : An abutment acts as the "stump" that holds the crown or false tooth that is screwed into the implant posts.


Crown : The crown is the visible false tooth of the implant restoration. Crowns on implants resembles a crown on natural tooth in almost every way. Crowns on implants can be porcelain fused to metal crowns or full ceramic crowns.




Reasons to replace missing teeth


There are many reasons why it's important to replace missing teeth:


Having all of your teeth can make you more self-confident. You don't worry that people notice that you have teeth missing.
When teeth are lost, the area of the jawbone that held those teeth starts to erode. Over time, you can lose so much bone that your jaw will need a bone graft to build up the bone in your jaw before your dentist can place implants or make a denture that fits properly.
Tooth loss affects how well you chew and what foods you are able to eat. Many people who have missing teeth have poor nutrition, which can affect overall health.

In 1952, a scientist named Per-Ingvar Brånemark was using titanium chambers screwed into bones as part of his research to discover how bone healed after an injury. When he tried to remove the titanium chambers, he found they had become bonded to the bone. This discovery led Dr. Brånemark to do further research into how titanium implants might work.




Dental Implants Versus Alternatives 


Depending on your particular problem, implants can be more expensive than the alternatives (dentures or bridge).


While the upfront cost for implants can be more than for other types of restorations, the investment can pay off in the long run. You do not necessarily need an implant for every missing tooth. Types of Dental Implants


There are three basic types of implants available at our dental clinic. In certain cases, conventional or immediate implants may be followed by immediate loading of crowns and bridges known as immediate loaded implants.

The type of dental implants selected will be dependent on the patients' specific needs and general dental condition. A panoramic x-ray and/or CT scan is usually required to evaluate the amount of bone, determine the space available and for a thorough examination to diagnosing the options best suited for each case.

Who Is an Implant Candidate?

If you're interested in replacing your missing teeth, or a single missing tooth, dental implants may be an option for you. Not everybody, however, is a good candidate for implants.

A good candidate should have the following:

• Healthy, disease-free gum tissues The remainder of the teeth and gums needs to be stable in order to put in dental implants which are likely to work. If there are other areas of the mouth with dental decay, broken teeth, or gum disease these will need to be treated first before implants are placed.

• Enough dense, healthy jawbone in a mouth Without the right amount of bone in the right place, it is difficult to place implants. In cases where sufficient bone is not available a range of grafting techniques can be used.

• Keeping the gums and bone around the Implant healthy The success of implants can depend on the ability of the patient to keep their teeth and gums clean. We need to be sure this will take place before implants are placed. We will always advise you on how to care for your mouth to allow you to have implants, and also how to care for your new teeth. Your commitment will be to follow our advice and should return regularly to your dentist for checkups, because any problems that might threaten the health of the implant must be corrected.

Some people may not be good candidates for implants.
They include:

Young patients whose jawbones have not developed completely
Pregnant women
Heavy smokers — Smoking impedes healing in the mouth and can reduce the likelihood that implants will be successful.
Alcohol or substance abusers
Patients who have received high-dose irradiation of their head or neck
People with chronic diseases or systemic problems, including high blood pressure, diabetes, connective-tissue diseases, hemophilia and significant immune deficiencies, among others — You still may be a good candidate for implants even if you have one of these conditions. It depends on the extent and severity of the condition.
Patients who take certain medications such as steroids or drugs that suppress the immune system
People who severely grind or clench their teeth — These habits can place too much pressure on the implants and increase the risk of failure.

Evaluation Process

Implant therapy involves a team that includes the surgeon (usually an oral surgeon or a periodontist) who places the implant or implants, and the restorative dentist (a general dentist or prosthodontist) who specializes in making crowns and bridges. The restorative dentist will make the crowns, bridges or dentures that will be supported by the implant or implants. The first step in your treatment is to make an appointment with us for an evaluation. 

Your initial evaluation will include an examination of your mouth and teeth and a thorough review of your medical and dental histories. You mouth will be X-rayed, and you might have a computed tomography (CT) scan, which will provide information on your bone density (how much bone you have in your jaw) and the shape of your jaw.

Finally, we will discuss the options available to you, talk about the procedure, and its cost and possible complications. We will work with you to develop a treatment plan that is customized to your particular needs and preferences.

    TIME REQUIREMENTS:

 We need one appointment for the procedure of placement of implants. We place all the implants in the same appointment. Afterwards, the final teeth can be placed either immediately or after a wait of 10-12 weeks.


This decision depends upon the clinical and radiographic assessment of the bone condition, the area of mouth where the implants are to be placed and the general medical history of the patient.


Generally, if we decide to go for the immediate loading implants, we can finish all the procedures in a week’s time. Otherwise, the patient needs to return for a follow-up visit for the placement of final teeth. This time for the second visit is at minimum 8-10 weeks time but can be after 3 months, 6 months or one year or longer depending upon the patient’s convenience.

What is implant dentistry?

Is dental implant surgery for me?

Can Anyone Get Dental Implants?


In most cases, anyone healthy enough to undergo a routine dental extraction or oral surgery can be considered for an implant procedure. Patients should have healthy gums and enough bone to hold the implant. They also must be committed to good oral hygiene and regular dental visits. Heavy smokers, people suffering from uncontrolled chronic disorders – such as diabetes or heart disease – or patients who have had radiation therapy to the head/neck area need to be evaluated on an individual basis. If you are considering implants, talk to your dentist to see if they are right for you.


Is the concept of dental implants a new one?


A Modern implantology began in the United States at the beginning of the 20th century. However, popularity really grew in the 1980’s with the increased success of the titanium cylinder. Since then, many brand name implants with minor variations have been approved.

When is the bone grafting required?


If your jawbone isn't thick enough or is too soft, you may need bone grafting before you can proceed with dental implant surgery. That's because the powerful chewing action of your mouth exerts great pressure on your bone, and if it can't support the implant, the surgery to replace your teeth would likely be a failure. A bone graft can create a much more solid base for the implant.


With bone grafting, a piece of bone is removed from another part of your body, such as your hip, and transplanted to your jawbone. The transplanted bone will grow, but it may take six to nine months to grow enough new bone to support a dental implant. In some cases, you may need only minor bone grafting that can be done at the same time as the implant surgery. The condition of your jawbone determines how you can proceed.




What happens after dental implant surgery?


Whether you have dental implant surgery in one stage or multiple stages, you may experience some of the typical discomforts associated with any type of dental surgery. These may include:


Swelling of your gums and face
Bruising of your skin and gums
Pain at the implant site
Minor bleedingVery rarely, stiffness of your jaw muscles may occur, or an inability to fully open your mouth. When these do occur, they're usually a result of passing a surgical needle through jaw muscle.
If swelling, discomfort or any other problem gets worse in the days after surgery, contact your implant surgeon. He or she may prescribe pain medications or antibiotics.


After each stage of surgery, you may need to eat soft foods for five to seven days. Typically, stitches that dissolve on their own are used. If your stitches aren't self-dissolving, your doctor removes them in about 10 days.


How do I Make my dental implant surgery a success?


Most dental implants are successful. Still, you can help your dental work — and remaining natural teeth — last longer if you:


Practice exceptional oral hygiene. Just as with your natural teeth, implants, artificial teeth and gum tissue must be kept clean. Specially designed brushes, such as an interdental brush that slides between teeth, can help clean the nooks and crannies around teeth, gums and metal abutments.
See your dentist regularly. Schedule dental checkups every six months to one year to ensure the health and proper functioning of your implants.

When dental implant surgery doesn't work?


In some cases, dental implants don't work. Usually that happens when the bone fails to fuse sufficiently to the metal implant cylinder. In this case, the implant is removed, the bone is cleaned up, and you can try the procedure again in a month or two.


In addition, the implant may become loose. If this happens, the implant can be removed and replaced with a new one.


What factors contribute to the long term success of dental implants? 


A Long-term success depends on multiple factors. Firstly, success will depend on the quality and quantity of one. The better the bone and the more available, the greater the chance of long-term success. Secondly, the experience and ability of the dental surgeon will be a factor. As with any surgical procedure, there is no substitute for the experience and individual talent of the dentist. And finally, the quality of the restoration placed on top of the implant will play a big role in long-term success. If the design of the implant crowns or overdentures are poorly constructed, and biting forces are not balanced, even the best-placed dental implant will have a compromised survival rate.


How Painful Are Dental Implants?


Most people who have received dental implants say that there is very little discomfort involved in the procedure. Local anesthesia can be used during the procedure, and most patients report that implants involve less pain than a tooth extraction. After the dental implant, mild soreness can be treated with over-the-counter pain medications


Are there any age limitations for dental implants?


No. Any person at any age can have dental implants as long as there is enough bone available in which to place the implants.


How often will I need to have my implants checked?


The success of your implants will depend greatly on how well you maintain them. They will need to be professionally cleaned by a hygienist and examined by your implant dentist every three to four months. This hygienist should be trained in the specific procedure of maintaining dental implants. Also, brushing and flossing daily is absolutely necessary for long-term success.


When can I return to work after implant surgery? 


You can go to work the next day, unless some particular surgical circumstance arises. Your implant dentist will discuss all postoperative instructions with you. 


Will my implant teeth work as well as natural teeth?

There are many different brands of implants with some being more expensive than another. In general, implants are not covered by dental insurance in USA at this time. Coverage under your medical plan may be possible, depending on the insurance plan and/or cause of tooth loss. Detailed questions about your individual needs and how they relate to insurance should be discussed with your dentist and your insurance provider. Placing a dental Implant
The first step is the placement of the titanium dental implants into the jawbone. Usually, the office procedure takes about an hour for one implant and no more than two to three hours for multiple implants. You may be offered sedation or intravenous medications. Then, a local anesthetic will be administered to numb the areas where the implants will be placed. After you are comfortable, a small incision is made into the gum tissue, revealing the bone into which the implant will be placed. Using special instruments, a socket is created, carefully avoiding damage to the bone. The titanium dental implant is then inserted into the socket. Finally, if necessary, sultures will be used to secure the incision.
The dental implants will remain covered underneath the gum for approximately 3 to 6 months. During this time, the jawbone bonds with the implant. This process is known as osseointegration. below provide more details below.


During the healing process, a temporary tooth replacement option can be worn over the implant site
1. An incision is made in the gum where the implant will be placed.
2. A hole is drilled in the bone.
3. The implant is placed into the hole in the bone.
4. The incision is stitched closed.
5. After healing, a new incision is made over the implant and another component, called an abutment, is screwed on top. The abutment is used to support a crown. A temporary abutment, called a healing abutment, usually is placed before a final abutment.
6. The final step of the process is the creation and placement of your new teeth, called crowns. Depending on the number and complexity of your crowns, this may require several fittings. So it may take one or two months to complete the restoration process. With cosmetic dentistry techniques that ensure optimal size, shape, color and fit, they will blend with your facial characteristics, while providing support needed for a natural appearance.The crown, which is made to look like a natural tooth, is cemented or screwed onto the abutment.

If you are missing a single tooth, one implant and a crown can replace it. A dental implant replaces both the lost natural tooth and its root.

A dental implant provides several advantages over other tooth replacement options. In addition to looking and functioning like a natural tooth, a dental implant replaces a single tooth without sacrificing the health of neighboring teeth. The other common treatment for the loss of a single tooth, a tooth-supported fixed bridge, requires that adjacent teeth be ground down to support the cemented bridge.

Because a dental implant will replace your tooth root, the bone is better preserved. With a bridge, some of the bone that previously surrounded the tooth begins to resorb (deteriorate). Dental implants integrate with your jawbone, helping to keep the bone healthy and intact.
In the long term, a single implant can be more esthetic and easier to keep clean than a bridge. Gums can recede around a bridge, leaving a visible defect when the metal base or collar of the bridge becomes exposed. Resorbed bone beneath the bridge can lead to an unattractive smile. And, the cement holding the bridge in place can wash out, allowing bacteria to decay the teeth that anchor the bridge.




Replacing Several Teeth
If you are missing several teeth, implant-supported bridges can replace them. Dental implants will replace both your lost natural teeth and some of the roots.

In addition, because implant-supported bridges will replace some of your tooth roots, your bone is better preserved. With a fixed bridge or removable partial denture, the bone that previously surrounded the tooth root may begin to resorb (deteriorate). Dental implants integrate with your jawbone, helping to keep the bone healthy and intact.
In the long term, implants are esthetic, functional and comfortable. Gums and bone can recede around a fixed bridge or removable partial denture, leaving a visible defect. Resorbed bone beneath bridges or removable partial dentures can lead to a collapsed, unattractive smile. The cement holding bridges in place can wash out, allowing bacteria to decay teeth that anchor the bridge. In addition, removable partial dentures can move around in the mouth and reduce your ability to eat certain foods.

If you are missing all of your teeth, an implant-supported full bridge or full denture can replace them. Dental implants will replace both your lost natural teeth and some of the roots.

Postcare Instructions for Dental Implants


Only liquid foods may be consumed during the first day in order to avoid inadvertent food particles contaminating the wound and semi-liquid (Mashed) food added during the rest of the week
Avoid eating or drinking anything very hot or cold during the first day after the operation
Moderate post-operative swelling is a normal reaction. However, If you have excessive swelling, severe pain or persistent bleeding please contact the clinic
Use antiseptic mouth wash rinses as directed
Smoking and consumption of alcoholic beverages should be avoided
Old dentures may not be worn until adjustments have been made. Early insertion of dentures may delay or affect healing
Avoid putting heavy pressure around the implant site during healing period



Care for Dental Implants


Dental implants require the same care as real teeth, including brushing, flossing, and regular dental check-ups:


Brush after eating and before bedtime around the implant with a soft toothbrush
Floss at least once to twice a day. Use the proxy brush or floss threader to remove plaque under and around these areas to maintain good oral hygiene. Buildup of food debris and plaque on your teeth and gums can become infected.
Rinse with fluoride rinse before bed. Swish the fluoride rinse vigorously in your mouth for at least one minute. Do not swallow any of the rinse and do not eat or drink anything for 30 minutes
Be careful about chewing toffees, gum, grainy rolls and tough food in this area

Though dental implants are more preferred these days, the question about how long it shall last was on top of my mind. My sister recently lost her tooth in an accident.
